What to bring

You need to bring your Medicare card and the details of your usual General Practitioner (GP) and/or GP practice, if you have one. If you don't have a Medicare card then bring one form of identification, like your driver's licence or student ID.

After your test

After your test you must return home immediately and stay at home until you receive your test result. This is referred to as ‘self-isolating’. You must do this because there is a risk that you could have COVID-19 and infect other people. Do not go to work or to the shops.

For GSHS Staff and visiting specialists, if you work at more than one site or you work for another employer, you are required to complete the "One Site, One Worker Declaration" and advise where you've worked previously before beginning your shift.

QR Check-in

Everyone entering a GSHS facility (this includes Leongatha Hospital, Korumburra Hospital, Koorooman House, Alchera House and Hillside Lodge) must check in via the QR codes displayed or via the below button.

The questions are recommended by the Victorian Department of Health for public health services and will assist us in contact tracing.  This QR Check-in is not linked to the Services Vic app as we are required to ask the standard COVID questions.
