An exciting opportunity exists for a nursing professional to work 0.21 EFT on a permanent part-time basis, within the Allied Health team at GSHS. This nursing position is responsible for the assessment and care planning component of the IMPACT program.


The successful applicant will possess an understanding of chronic disease management principles and have excellent global health assessment including medication management knowledge. Key skills of the successful candidate are care planning and coordination with internal and external health professionals.


Candidates must demonstrate relevant professional development in related areas of health and wellbeing including the impact of mental health and substance abuse in chronic health conditions. The successful candidate will require excellent computer skills to ensure accurate reporting and data collection and will also need a Working With Children Check and NDIS Worker Screening Check.
