Support at Home

The Support at Home Program is funded by the Commonwealth Government.  Support at Home is a program to help older people to remain living in their own home for longer, by providing a range of services to assist their assessed care needs.

 

To be eligible for the Support at Home program, an older person/participant must be assessed and approved under the Aged Care Act 2024 by My Aged Care through the single assessment system.

 

GSHS Support at Home

Gippsland Southern Health Service pride themselves on delivering Support at Home services under a person centred care model, working in partnership with the older person/participant to understand their individual needs, wants and goals, delivering the services to meet those needs. 

 

Services funded under Support at Home

Ongoing Support at Home funds can be used for a wide range of services including:

  • Clinical Supports – such as nursing care and allied health (e.g. podiatry, physiotherapy, dietician, occupational therapist, speech pathology, social work).

  • Independence – such as personal care, social support (individual or group), respite care, community engagement and transport

  • Everyday Living – such as domestic assistance, home maintenance and repairs and meals.

 

In addition there are 3 short term support classifications that can be funded under Support at Home:

  • Restorative Care Pathway – A short‐term, goal-oriented allied health and/or nursing intervention to help a participant regain or maintain independence and reduce the need for ongoing supports.

  • End of Life Pathway – A high-priority support pathway for older persons assessed as having three months or less to live, enabling additional in-home aged care and associated supports to allow them to remain at home with dignity.

  • Assistive Technology and Home Modifications (AT-HM) – Funding is provided for equipment, products and home adaptations to increase safety, accessibility or independence at home.
