Community Allied Health Team


The Community Allied Health Team (CAHT) aims to help people with health issues to stay in their own home where it is possible and practical.The team provides services that would otherwise be unavailable to people due to their disability, frailty, rural isolation, financial or other difficulty.
The services are offered to:

The elderly

People with a disability
-
Carers of these people
-
Professionals or community organisations working with consumers and their carers
The services provided are:

Dietetics

Physiotherapy
-
Podiatry
-
Speech Pathology
-
Occupational Therapy
The Community Allied Health Team works together with HACC services such as District Nursing, as well as Shire based services and other community based care providers.
The cost?
There is a small fee associated with the Community Allied Health Team services.
The Community Allied Health Team can assist you by:

Showing you ways to be more independent and safe at home.

Providing carers with information and assistance.

Offering home based support including advice, treatment, referral, and recommendation of equipment.

Providing education and preventative health programs.
-
Providing assistance to local health and support workers.
Information on our services can be provided to you individually, or to community groups such as Senior Citizens or support groups.Topics and information provided include:

Swallowing disorders

Voice and hearing impairment

Diabetes and therapeutic diets

Referrals are welcome from anyone.
