WHO IS THE FALLS PREVENTION PROGRAM FOR?
Anyone who has had a fall or is at risk of falling. Each participant is individually assessed for suitability prior to commencing the program.
Place: Gippsland Southern Health Service Leongatha Hospital Primary Health - Physiotherapy Department
Time: Wednesday 2- 4p.m.
Length of Program: 8 weeks
Carers are welcome to attend.
Afternoon tea is provided (pending COVID-19 restrictions), along with a take home booklet which includes the exercise program.
Transport to and from the group can sometimes be arranged.
AIMS OF THE PROGRAM
Standing up to Falls is an exercise and education program for anyone who has had a fall or is at risk of falling. The program aims to:
Provide you with information about preventing falls
Help identify and reduce your personal risk factors for falls
Enable you to participate in exercises to improve your strength and balance
Participants meet weekly for the program.
Each week, a physiotherapy-led exercise session is combined with an education session from one of the following health professionals: Physiotherapist, Occupational Therapist, Pharmacist, Dietician, Continence Nurse, Podiatrist, Vision Australia.
At the end of the eight-week program, you will be offered the opportunity to become involved in other exercise and social groups within Gippsland Southern Health Service and the wider community. Continued participation in effective exercise programs ensures the best outcomes.
REFERRALS TO THE FALLS PREVENTION PROGRAM
The Falls Prevention Program operates at the Leongatha Campus of Gippsland Southern Health Service on Koonwarra Road.
Referrals can be made by your doctor, allied health professionals and community health professionals
You can also refer yourself to the group or have a family member contact us on your behalf.
